Please use the information below as a guide to assist you as you begin your career
with The University of Toledo.

Pre-Hire Requirements
The following steps should not be completed until an official verbal offer has been
made by a Human Resources Employment Advisor. At the time of the verbal offer you
will be instructed with regard to all pre-hire requirements you must complete. The requirements must
be completed prior to your first day of employment.
All New Employees
1. New Hire Forms
2. Receipt of your official transcripts verifying your college degree(s) required
for the offered position in a sealed envelope mailed from the college/school directly
to the following address:

3. UT Medical Center and College of Medicine employees, and all employees who will physically
be working on the Health Science Campus ONLY.
To complete drug testing and employee health screening requirements please contact
the University Health Services/Family Medicine Clinic at 419-383-5555 or 419-383-3777 to
schedule an appointment. The Family Medicine Clinic is located on the Health Science
Campus, Ruppert Health Center, Suite M. You may park in lot 48 directly outside the
building. You will need to bring any immunizations records you may have with you

Beginning Employment
Orientation - Information at this link must be reviewed prior to orientation.
API Training (Timeclock) - Training is scheduled for the day following orientation from 8:30am - 10:30am,
located on the Health Science Campus. Please contact your Employment Advisor for
the building and room number as these sometimes vary by orientation. This training
is mandatory for all hourly employees working on the Health Science Campus. *Nursing positions - Due to a conflict with nursing orientation times, you will be scheduled for a separate
API training by your department.
E-mail/UTAD access - Once your paperwork has been entered into the Banner payroll system your UTAD access
and e-mail address will be automatically generated. To activate your UTAD and e-mail
account please click here and follow the instructions.
W4 Federal Tax Exemption or Allocations - Once you have activated your UTAD account above you may go online to My UT to update your exemption or allocations. Please click here for instructions.
For assistance in determining whether you are eligible for exemption or the number
of allocations to elect please click here and use the Personal Allowances and Deductions and Allowances Worksheets.
Direct Deposit - Once you have activated your UTAD account you may go online to My UT to sign up to have your check direct deposited into the account(s) of your choice.

ID Badge - Once you have activated your UTAD account you may go online to My UT to request your ID Badge. Click on the Employee Tab - My Toolkit - ID's. Please
click here for further instructions. Please contact Auxiliary Services at rocketcard@utoledo.edu or 419-530-5842 for questions and issues regarding this process. There is no charge
for an employee's first ID Badge.
