HSC Card Access Request

To request door access at the University of Toledo on Health Science Campus, the dean, director, department head, manager, supervisor, or lead staff person who is above the individual for whom access is being requested, must email a completed request form to:

HSCCardAccessRequest@utoledo.edu

The online request form can be accessed by clicking HERE. Please take a moment to read these tips BEFORE completing and submitting your request:

1. If this is your first time applying for a door access on HSC your supervisor must also email a current head shot photo along with the completed request form. All photos must be free of anything that would obstruct the view of the face -- no sunglasses or hats are allowed in the photo. Photos not meeting these requirements will be rejected.
   
   
2. If you need a photo, and are associated with Facilities can obtain assistance at the Facilities Support Building (FSB) on the Health Science Campus between 8 a.m. and 4 p.m., Monday to Friday. Anyone else needed a photo should visit Auxiliary Services in the basement of Mulford Library (room 1000) between 8 a.m. and noon and 1 and 4 p.m., Monday to Friday.
   
   
3. The form MUST be emailed from the appropriate approver (dean, director, department head, manager, supervisor, etc.) to the listed email address.
   
   
4. The requestor will be contacted as soon as the door access request has been completed. Individuals receiving an access card for the first time will have to pick them up in the basement of Mulford Library, room #007. You must display a current UT ID when you come to pick up your access card.
