    GME Resident Agreement

    Resident Salary Levels (effective July 1, 2010)

    Initial Resident Credentialing File

    A completed resident credentialing file must consist of the following:
    • A completed application form (either ERAS or Universal) - original or copy
    • Current resume - original or ERAS copy
    • Dean's letter - original or ERAS copy
    • Two (2) letters of recommendation - original or ERAS copy
    • Transcript - original, must contain the Registrar's signature and seal
    • Medical school diploma - notarized copy
    • Certificates of previously completed residencies - notarized copy
    • Non-US citizens must have an active I-94 or Permanent visa - notarized copy
    • International Medical Graduates must have a valid ECFMG certificate - notarized copy
    • USMLE report documenting successful completion of USMLE Step 1, Step 2CS, and Step 2CK
