GME Resident Agreement Resident Salary Levels (effective July 1,
2010)
Initial Resident Credentialing FileA completed resident credentialing file must consist of the following:
- A completed application form (either ERAS or Universal) - original or copy
- Current resume - original or ERAS copy
- Dean's letter - original or ERAS copy
- Two (2) letters of recommendation - original or ERAS copy
- Transcript - original, must contain the Registrar's signature and seal
- Medical school diploma - notarized copy
- Certificates of previously completed residencies - notarized copy
- Non-US citizens must have an active I-94 or Permanent visa - notarized copy
- International Medical Graduates must have a valid ECFMG certificate - notarized copy
- USMLE report documenting successful completion of USMLE Step 1, Step 2CS, and Step 2CK
