Veterans Request Form - this form must be completed each semester in order to receive benefits.
Request for Change of Program or Place of Training VA Form 22-1995 - this form is to be filled out if you have changed your major or degree of study.
Dependents Request for Change of Program or Place of Training VA Form 22-1995 - this form is to be filled out by dependents using benefits who have a change of major or degree of student.

New Students Forms

Application for VA Education Benefits VA Form 22-1990 - this form is to apply for VA benefits for new students.
Veterans Responsibility Form - this form must be filled out by all new students.
Veterans Request Form - this form must be filled out each semester in order to receive benefits
Verification of Attendance Policy - this form must be filled out by all new students as an understanding of attending classes.
Post 9/11 Student Responsibility Agreement - this form must be filled out by all students using Post 9/11 benefits in regards to tuition and fees.
