How can I obtain an ALI application form?
- You can write to us and we will send you an application packet, or
- You can print the ALI Application Form (54k Adobe PDF) from this website.
When your application is complete, be sure to:
- Sign it
- Have your sponsor (parent, embassy, company) sign it
- Enclose the $40 application fee (check or money order)
- Enclose the bank letter or financial guarantee letter. bank letter- sample (3k Adobe PDF)
- Send us the application by fax, mail or courier (Note: if you fax it, you must also send by mail the original signed application, $40 fee and bank letter).
