New Faculty Hire
The original forms, no copies, are required to be completed as a new faculty member at The University of Toledo. Upon completion of forms, please return to the department chair in the college department of hire. Your offer letter will designate where to return documents.

Required Forms
- Background Check Authorization
- International Background Search - If a candidate is a non-U.S. citizen, this form
will also need to be completed.
- Transcript - original transcript of all degrees. Copies are NOT accepted.
- I-9 Employment Eligibility Verification - I-9's are processed electronically through
the Intellicorp system. Your hiring department will assist with completing this process.
Original documents must be presented in-person to the department of hire. I-9 Instructions.

- STRS Notice of Re-Employment as a Teacher (Form 50-150) - To only be completed by rehired retirees
- Statement Concerning Your Employment in a Job not Covered by Social Security (SSA-1945)
- W-4 Federal Tax Withholding Online Instructions - Completed through MyUT Portal
- State Tax Withholding Form (Michigan or Ohio, not both) Ohio Form ; Michigan Form
- Ethical Conduct Statement - Before completing the form, please review the Ohio Ethics Law and Related Statutes
