In order to apply for the pharmacy summer camp, you must complete the on-line application.
A grade verification form must be completed, signed, sealed and sent in by your high school guidance counselor. BOTH must be completed and submitted on time in order to be considered for admission to the camp.

Begin by registering for an account. Once an account is created, you can log in and submit your application. Login later to check your status or print your application.

• Grade verification form postmarked by: April 1, 2013
• Acceptance status: Available online on Apr 15, 2013
• If selected, camp reservation confirmation, paperwork and fee due: May 6, 2013
