Spring Plan of Degree Coursework

Fall Plan of Degree Coursework
This form will be used to monitor this student-athlete’s degree progress in their academic program. The information provided will assist SAAS staff in tracking this student-athlete’s NCAA eligibility and progress toward graduation.

Extra Hours Authorization Sheet
In order to receive coverage of extra credit hours, student-athletes must completes the Extra Hours Authorization Sheet prior to the end of add/drop period.

Tutor Evaluation Form
Tutors are required to complete a Tutor Evaluation Form for the record after each tutoring session.

Travel Memo
Access your sport travel memo by clicking on the link below. Student athletes should print out a copy to share with each of their faculty members at the start of the semester alerting them to team travel dates.
