 Thank you for visiting our Equipment Request Page. 

 All the various pieces of equipment Central Distribution (stores) manages are displayed below.   Click on the type of equipment, enter the appropriate information, submit the form and your equipment will be delivered shortly thereafter by the operations team.

 Special note:  If you are in need of a breath/blow unit that integrates with our nurse call system, please call x 3488 (Work Control) M-F during 1^st shift hours.   For 2^nd and 3^rd shift, weekends and holidays, please call X 4298 (Central Control) and they will deploy one to you.     This item is not managed by Central Distribution.
