Judith Herb College of Education: Ohio Administrative Licensure Application Process
1. Students should initiate the application process for licensure very near the end of all required coursework. Begin the process by calling the College of Education Office of Student Services (419-530-2495) to request the required ODE application and other required forms (see #3 below). Additional and current information on state licensure is available on the ODE web site.
2. Students must have taken and passed the Praxis III examination for administrators. To be sure official results of your test are sent directly to The University of Toledo, use the following code in your test application: R1845.
Only official copies of results will be accepted. For information on the Praxis exam, including sample questions, visit www.ETS.org.
3. After taking the Praxis exam, students will need to:
• Complete a “transmittal form”
• Complete a verification form for teaching and/or administrative experience which must be signed by a district representative
• Complete the ODE application form (see #1 above)
• Complete a fingerprint and FBI background check which can be done through the UT Police
• Pay a processing fee (cash or check made payable to: Ohio TEC)
Forms listed above and information on the background check process are available through the Student Services Office, Suite 3000, Gilham Hall.
4. Submit all the documents above to the Student Services office. (Praxis results will come directly to the College if you request it and use the code R1845). When all components required for application are received by the College, your transcripts and application materials are reviewed by Leadership Department and College staff. If everything is in order, your application is forwarded to the Ohio Department of Education with a recommendation to grant administrative licensure. (If it is not, you will be contacted by the Office of Student Services.)
Plan for the entire process to take up to ten (10) weeks!
