Graduate Curriculum Forms and Submission Process 

To submit graduate curricular proposals, please use the PDF fillable forms provided here.  Complete the form, print, obtain all required signatures and send to Terri Hayes in College of Graduate Studies (mail stop 933).  Please be sure to attach additional information as necessary, i.e. (syllabus, etc).  Material will be accepted anytime, but may take longer to process in the summer.

 * It is recommended to download the form to your desktop/laptop to use it. The PDF viewer in Google Chrome Browser does not support several Adobe Reader features and Internet Explorer/Mozilla Firefox are also known to have some issues while trying to save the document in the browser.
