Application Materials

Applications for the Human Donation Science program are processed through the College of Graduate Studies Admissions Office. Applications are only accepted using a standard online application form.

After submitting the online application please mail the following materials to the College of Graduate Studies Admissions Office in one packet.

• Three letters of recommendationRecommendation Form - 
  Print Friendly PDF / Online Fillable and Submittable PDF
• $45 Application fee
• Letter of Intent / Application Essay

All official college/university transcripts and official GRE and TOEFL results must be sent directly from the respective college/university and GRE organization to the College of Graduate Studies, University of Toledo.
