Online Affiliation Verification
MDQuery is an online hospital affiliation verification system that allows you to query UTMC privileged providers and print a verification letter.
- FOR BEST RESULTS: Once you are on the website, please enter only the "Last Name " of the practitioner. If you cannot locate a provider or have questions, please contact us at (419) 383-5322 or at MedStaff@utoledo.edu If your Provider is not found, and that provider completed a training program at UTMC, please contact the GME office at GMEOffice@Utoledo.edu, or (419) 291-8201.
- In the event you are unable to obtain the information requested from our website, please email a signed release with your request to MedStaff@utoledo.edu and include a copy of the message showing no results found.
