Contact Fleet Operations: UTFleetOps@utoledo.edu to request a fuel key fob. Include in the email the name of the department, index, and the driver name(s).
- Fuel fobs are limited to one (1) per vehicle.
- Initial fuel key fob no charge.
- Non-working fuel key fobs are replaced at no charge.
- Stolen fuel fobs replaced at no charge with submitted police report.
- Lost/Damaged fob replacements fee $10
Fuel cost per gallon will be billed to responsible index monthly and will be based on the market cost per gallon at the time of fuel delivery, plus a 10% fee to cover the cost of fuel delivery, administrative costs and pump/tank maintenance.
