This form is for University of Toledo students, residents, fellows, employees, and approved affiliates who need written confirmation of medical professional liability coverage for authorized academic, clinical, or training activities.
Coverage is provided through the University’s medical professional liability program and, when applicable, the University of Toledo Medical Assurance Company (UTMAC).
