Par Change Request
Supply chain will review and make the necessary adjustments to PAR locations. The business owner of the index/PAR location (nursing director, department manager, clinic manager, etc…) are the persons who may submit these requests. This ensures we are not adding items not approved for use by clinical management.
If a person other than the business owner places a request, we will forward to the business owner/copying in the requestor and no action will be taken until authorized by the business owner.
When requesting an item to be ‘added’, the item must already reside onsite in one of our inventory warehouse locations (central distribution, surgical support or cath lab). If the item does not reside onsite, then close out this form and go to the PRODUCT/ITEM ADD request form.
Once the par change request has been completed, you will receive a communication of completion of this request.
