Incident Reporting

Reporting an Injury or Auto Accident - Non-Workers’ Compensation incidents only. 

This page is for reporting injuries, incidents, and auto accidents that are not related to Workers’ Compensation. In certain situations, such as an auto accident involving a University employee, both this reporting process and Workers’ Compensation reporting may be required, as explained below.

Use this form if the incident involves:

  • Students
  • Visitors or guests
  • Volunteers
  • Contractors (non-employees)
  • Members of the public
  • University property damage
  • Auto accidents involving University vehicles
  • Auto accidents involving third parties

What to Do:

  1. Ensure safety and obtain medical care if needed, dial 911
  2. Contact campus police or security (non-emergency) to file a report
    • Main Campus University Police (Non-Emergency): 419.530.2600
    • Health Science Campus Security (Non-Emergency): 419.383.2600
  3. Complete the University of Toledo Accident / Incident Report (Non-Workers’ Compensation)

Employees Involved in Auto Accidents — Important Clarification

If a University employee is involved in an auto accident while performing work duties, both the Incident Report Form and notification to Workers' Compensation may be required.

What an Employee Should Do:

  1. If you are injured: Report the injury through Workers’ Compensation
  2. Regardless of whether you are injured: Complete the University of Toledo Accident / Incident Report (Non-Workers’ Compensation) for the auto accident and submit it to Risk Management.
