Report an Injury or Auto Accident - Non-Workers’ Compensation Incidents Only
This page is for reporting injuries, incidents, and auto accidents that are not related to Workers’ Compensation. In certain situations, such as an auto accident involving a University employee, both this reporting process and Workers’ Compensation reporting may be required, as explained below.
Use this form if the incident involves:
- Students
- Visitors or guests
- Volunteers
- Contractors (non-employees)
- Members of the public
- University property damage
- Auto accidents involving University vehicles
- Auto accidents involving third parties
What to Do:
- Ensure safety and obtain medical care if needed, dial 911
- Contact campus police or security (non-emergency) to file a report

Employees Involved in Auto Accidents — Important Clarification
If a University employee is involved in an auto accident while performing work duties, both the Incident Report Form and notification to Workers' Compensation may be required.
What an Employee Should Do:
- If you are injured: Report the injury through Workers’ Compensation
Health Science Campus Employee
- Regardless of whether you are injured: Complete the Non-Workers’ Compensation Accident / Incident Report for the auto accident and submit it to Risk Management.
