My name does not show up on the request form.
- Complete this form: Add New User to Key Database
- Key Control will add your name and permission to request keys
How long before I receive my key?
- Keys will be ready for pick up within 48 hours after approval from Locksmith.
- You will receive an email notification when keys are prepared, and a separate email from KeyCafe.com with an 8 digit code to access your key from our pick up lockers.
- Keys are not available for pick up until you receive the 8 digit code.
Can Someone Pick Up My Keys For Me?
- While keys should be picked up by the assigned key holder, keys can be accessed by anyone with your 8 digit code.
How do I Return My Keys to UToledo?
- Main Campus - Plant Ops building: M-F from 7:30 a.m. - 3:30 p.m. – key drop box
- Health Science – Facilities Support Building; M-F from 7 a.m. - 3 p.m. – key drop box
How to Submit a Key Request
- Click the "Request a Key" link.
- Log in with UTAD credentials.
- Click blue "REQUESTS AND ORDERS" link at top of page.
- Click the blue "New Key Request".
Where Do I Pick Up My Keys?
- Keys are available for pick up thru a code accessed key locker.
- You must have your 8 digit access code to open your locker
- Main Campus: Plant Operations Building, Room 1090
- Health Science Campus: Facilities Support Building, Door 1 (Lot 44D)
How do I Report A Lost Key?
- Submit an online request.
- Click the "Request a Key".
- Log in with UTAD credentials.
- Click blue "REQUESTS AND ORDERS" link at top of page.
- Click the blue "New Key Request"
- In the "Reason" drop down box select Damaged, Lost, or Stolen
My Keys Are Not Working?

Who are the Approvers in the Department?
- Your direct supervisor will be notified of your submitted request, then it will go to the below individuals for approval:
- Deans
- Department Heads
- Director or Vice President
How Can I Be Added/Removed as a Key Approver?
- Department Heads are automatically Key Approvers.
- Department Assistant are automatically Key Approvers.

Can I Add a Proxy for Key Approval?
- There is no proxy capability in the new system.
- To add an eligible approver contact: keycontrol@utoledo.edu
Leaving the University? What Do I Do With My Keys?
- You and your supervisor should receive a list of keys assigned to you.
- Locate the keys and return to Key Control within 7 days of your separation date.
- If not returned, a fee will be assessed to the department.
Can I Transfer My Keys to My Replacement?
No, keys are assigned under your name and need to be returned by you in order to reassign them.
What are the Fines & Fees for Key Replacement, Feb. 2023?
- Unclaimed Key $25
- Re-issue of Lost Key $25
- Unreturned Key $25
- Re-Key $100 base cost*
- Re-Core $200 base cost*
- *Final cost dependent on labor & material costs and the number required.
Contractors: How Do I Request Keys?
- Key Request forms need to be completed during initial phase of the project.
- Our UT Project Manager will work directly with you and coordinate your access as part of the project.
- Fill out the information on the Contractor Key Request form.
- You will receive an 8 digit access code via the email your listed on your request form when your keys are ready.
I Need Swipe Access Added to my ID Badge

No, you will need to complete a maintenance work order.
