Affiliate Eligibility
What makes a site eligible to host a student at their facility or organization?
- Before an organization or facility can host a student, they must first have a valid academic affiliation agreement with the University of Toledo College of Health & Human Services. A new academic affiliation agreement can be initiated by completing a CHHS Affliate Agreement Request Form. If an organization/facility has their own agreement tempmlate, they can contact the Site Eligibility Coordinator at chhssites@utoledo.edu and he will initiate the review process with our Office of Legal Affairs.
- Once an academic affiliation agreement has been finalized, any appropriate practitioner employed by that organization may host a student. CHHS will have to verify that the practitioner holds the appropriate license/credentials within their field before that individual is permitted to supervise students.
