Elective Description/Requirements: The student will become a part of the emergency team whose members handle all phases of emergency patient management ranging from triage to final disposition. Students will evaluate and manage Emergency Department patients under the direct supervision of the EM faculty. Students will perform history and physical examinations, document findings in the medical record, order appropriate diagnostic and therapeutic interventions, assess response to therapy and determine appropriate patient dispositions. Students will conduct follow-up communications with select patients to assess short- term response to therapy and to ensure follow-up instructions are being followed and     assist with any problems under guidance of ED faculty and staff. Acting Internship students will be required to adhere to and be able to show competency   in the Emergency Medicine Student Milestones which will prepare students interested in EM as a specialty to the Milestones used by ER residency.

Length of Clerkship: 4 Weeks

Educational Course Objectives: At the end of this elective, the student will be able to:

  1. Obtain a focused developmentally appropriate biomedical and psychosocial story of the patients’ illness in the acute care setting. (EPOs: MK-1, MK-4, MK-6, MK-7, PC-1, PC-2, PC-5)
  2. Perform a physical exam that is focused on the patients’ acute problem. (EPOs: MK-1, MK-4, MK-6, MK-7, PC-1, PC-2)
  3. Communicate with patients, families, physicians, EMS personnel and ancillary staff. (EPOs: MK-1, PC-1, PC-4, PC-5, PC-9, PB-1, PBL-3, IPC-1, SBP-4, SBP-5, IPC-2)
  4. Interpret diagnostic tests that are pertinent to the acute care setting. (EPOs: MK-1, MK-2, MK-3, PC-1, IPC-1, IPC-2)
  5. Perform basic emergency medicine procedures, including incision and drainage. (EPOs: PC-5, PB-1, IPC-3)
  6. Care for acute injuries. (EPOs: PBL-1, PBL-3, SBP-3, SBP-4)
  7. Formulate management strategies, which are consistent with the acuity of the illness as well as the patient’s interests, needs, and preferences. (EPOs: PC-3, PC-5, PB-1, PB-3, PBL-2, SBP-1, SBP-2, SBP-4, SBP-6, SBP-4, MK-5, MK-8)
  8. Triage acute medical conditions in a timely manner. (EPOs: MK-1, MK-5, PC-3, PC-6, PC-7, PC-8)
  9. Demonstrate appropriate utilization of professional consultations in the acute care setting. (EPOs: MK-1, PC-6, PC-7, PC-8, SBP-1, SBP-5)
  10. Formulate safe and appropriate dispositions for a wide range in clinical conditions germane to the practice of Emergency Medicine. (EPOs: MK-1, PC-1, PC-6, PC-7, PC-8, IPC-1, SBP-5)
  11. Present clinical information, both in the verbal and written format, in a concise and coherent manner. (EPOs: MK-1, PC-4, PC-7, PC-9, IPC-1, IPC-2, IPC-3, SBP-5)
  12. Participate in evening or weekend clinical care to support continuity of patient care and demonstrate professionalism in varied clinical settings. (EPO: IPC-2)
  13. Conduct structured patient handoffs using an evidence‑based communication approach to ensure safe and accurate transfer of care. (EPO: SBP-1)

Professionalism: UT/COMLS students will meet or exceed the institutional standards for professionalism as stated in the current Educational Program Objectives and the current Educational Course Objectives for the Sponsoring Department.

Instructional Methods:

  1. Individual case management with faculty.
  2. Procedure lab and lecture
  3. Emergency Medicine didactic conference.
  4. Direct observation and interaction.
  5. Resources: Tintinalli’s EM, Mulford Library/website (UptoDate), Rosh Review, EM Clerkship guide.

Assessment Methods:

  1. M4 Acting Internship Assessment
  2. Attendance at Conference, Workshops, and ED clinical shifts as offered by program.
  3. Assessment of patient case logs, procedure logs, and hours.
  4. Any Final exam and course evaluation per institution

Prerequisites: Successful completion of third year and passing USMLE Step 1 score.

Special Requirements:

  1. As part of the acting internship, students are required to participate in at least one evening or weekend clinical experience.  This experience ensures exposure to the full spectrum of patient care responsibilities and supports readiness for residency-level schedules and work environment.
  2. Students are expected to participate in structured patient handoffs as part of their clinical responsiblities.  This includes observing and contributing to the sign-out processes to ensure safe, effective transitions of care.
