Elective Description / Requirements: MD/JD students will identify a clinical mentor. The process for selection of the mentor and approval of the MD/JD Student Clinical Training Agreement will be under the direction of the MD/JD Program Director(s). The clinical faculty mentor will be responsible for the clinical training program of the student and will provide formative and summative feedback concerning the development of clinical skills and a foundation for subsequent clinical clerkship training. The clinical mentor may change during the course of the student's JD years, but any change should occur after the end of a semester. Students may choose a clinical mentor from any department; the following specialties may be particularly suited to training MD/JD students who have limited prior clinical training and restricted hours of availability: Internal Medicine, Emergency Medicine and Family Medicine.

Student Time Commitment: Students are expected to spend 8 hours per month in clinical training. This can be divided into weekly 2-hour sessions, biweekly 4-hour sessions or an 8-hour day per month. Students are encouraged to discuss the most appropriate schedule with their law mentor to ensure that the clinical experience does not interfere with coursework or other progress. The program should be formalized in writing, and the MD/JD Student Clinical Training Agreement submitted to the MD/JD Director(s) for approval. Students should see at least one patient per hour of training. Students should keep a log of their patients, their diagnoses and any procedures performed.

Credit: Upon completion of 2 years of graduate training, assuming 12 months per year of participation, students will have accumulated about 190 hours of clinical experience. This is approximately equivalent to 4 weeks of clinical electives, so student will be awarded 4 weeks of clinical elective credit during 4th year upon re-enrolling in the MD program.

Educational Course Objectives:

  1. Develop and demonstrate history taking and physical examination skills. (EPOs: PC-1)
  2. Demonstrate how to present a cogent history and physical to an attending. (EPOs: IPC-1, PC-1)
  3. Present the differential diagnosis, diagnostic work-up and management for common medical problems. (EPOs: PC-6, PC-7)
  4. Relate patient presentations and treatments to basic pathophysiology and pharmacology. (EPOs: MK-4)

Professionalism: MD/JD students will meet or exceed the institutional standards for professionalism as stated in the current Educational Program Objectives and the current Educational Course Objectives for the Sponsoring Department.

Instructional Methods: Student interviews and examines patients, followed by presentations to faculty and clinical teaching on each case. Additional reading or write-ups at the discretion of the clinical mentor.

Evaluation methods employed: Students will undergo formative evaluation on a monthly basis, documented by a brief form signed by both the mentor and student. This feedback will not be used for grading purposes. Summative evaluation will occur twice yearly by the clinical mentor, filling out an evaluation form to be mailed to the Graduate School Office and the COMLS Office of Student Affairs. This form will assess Cognitive/Performance Skills (ability to perform H&P and present to the attending) and Professionalism (promptness and consistency of attendance, appropriate attire, etc.). Students repeatedly lacking in these areas will not receive elective credit for their rotation. The student will receive a copy of this evaluation and is urged to discuss any issues with the clinical mentor. Copies of the records of student participation and performance evaluations will be kept by the Graduate School Office and the COMLS Office of Student Affairs.

Prerequisites: Successful completion of all preclinical coursework and passing Step 1 of the USMLE examination.

Clerkship Director(s): Dr: Sohn will organize the rotation for all MD and JD students. The chosen Clinical Mentor will be responsible for the quality of the educational experience and evaluation of student performance.

Special Requirements: On-line logging of patient experiences and hours spent, as well as faculty evaluation of students.

MD/JD Student Clinical Training Agreement

As part of their training program, MD/JD students are expected to participate in a clinical training experience during the graduate school/research phase of their training. The goal of this experience is to reinforce preclinical information and help develop clinical skills that will be used when students transition back to the clinical clerkships after completing their J.D. degree requirements.

Learning objectives for this rotation are as follows:

  1. Develop and improve history taking and physical examination skills. (PC-1)
  2. Describe how to present a cogent history and physical to an attending. (PC-1)
  3. Present the differential diagnosis, diagnostic work-up and management for common medical problems. (PC-1, PC-7)
  4. Relate patient presentations and treatments to basic pathophysiology and pharmacology. (MK-4)

Students will work with a Clinical Mentor who will take responsibility for the clinical experience and evaluate student progress. The clinical mentor is not required to proctor all of the clinical exposures, but to ensure that the student is appropriately supervised and receives a good clinical experience. Students are expected to spend about 8 hours per month in clinical training. This can be divided into weekly 2-hour sessions, biweekly 4-hour sessions or an 8-hour day per month, or other arrangements as determined by the Student and Mentor. Students are encouraged to discuss the most appropriate schedule with their Research Mentor to ensure that the clinical experience does not interfere with graduate coursework or research progress. Students should see about one patient per hour of training (or more if possible), and should keep a log of patients seen, their diagnoses, and any procedures performed. Students will sign up for 1 or 2 hours of elective credit per semester, which will count toward the 4th-year clinical elective credit at the rate of 2 weeks of credit for every 3 credit hours earned in this program. Clinical Mentors should not change more than once per semester.

In the summer after the second year of medical school, MD/PhD students will identify a clinical mentor who will be responsible for overseeing clinical training for the student during a portion of his/her JD program phase, and will provide formative and summative feedback concerning the development of clinical skills and a foundation for subsequent clinical clerkship training. The mentor may change during the course of the student’s JD school years, but changes should occur after the end of a semester.

Clinical Setting and Training: Students schedule specific hours (which may include evenings or weekends) with their clinical mentor. Patients will be assigned to the student for evaluation, and students will be involved in all aspects of emergency patient care, under direction of their clinical mentor or assigned Emergency Medicine attending. Students should see about one patient per hour of training. Students are expected to document their patient encounters as appropriate and should be given feedback on these notes. Students should keep a log of their patients, their diagnoses, and any procedures performed. An electronic logging system for patient experiences will be developed.

Student time commitment: Students are expected to spend 8 hours per month in clinical training. This can be divided into weekly 2-hour sessions, biweekly 4-hour sessions or an 8-hour day per month. Students are encouraged to discuss the most appropriate schedule with their laboratory mentor to ensure that the clinical experience does not interfere with graduate coursework or research progress. If students wish to limit their commitment to 4 hours per month, with permission of the course director, they should only sign up for one credit for that semester.

Student/Mentor Contract: The student/mentor relationship should be formalized in a brief contract in which both acknowledge their time commitment, as well as the student’s research mentor (to ensure that the dissertation advisor understands that this is an essential part of the student’s training). Completed contracts should be submitted to the MD/PhD Director and to Pat Munier. The clinical mentor is responsible for ensuring that the student has a high-quality training experience but need not be the attending physician for all patient experiences. The mentor is responsible for providing a grade (submitted through the clerkship director) and both formative and summative evaluations.

Credit: MD/JD Students enrolled in JD program will sign up for 1 or 2 credit hours during the fall or spring term while enrolled in JD program and 1 credit hour during the summer term, consistent with an average of 8 contact hours per month (2 per week). Students may earn a maximum of 4 credit hours per academic year. For every 6 hours of credit, 4 weeks of fourth year clinical elective credit will be granted. In the course of 2 years of graduate training, students may earn a maximum of 6 credits and thus be eligible for a maximum of 1 month of clinical elective credit.
