Step 1. Gather student’s most recent grade card; Standardized Test scores & two Recommendations from a Teacher, Counselor or Mentor. [Adobe Scan offers a FREE .pdf creator to help with this.]

Step 2. DOWNLOAD, COMPLETE the UB/UBMS Declaration pages.

Step 3.  Select either the Upward Bound or the Upward Bound Math Science tab and complete the online application.

Step 4. Where prompted: UPLOAD the completed Declarations, grade card, Standardized Test scores and recommendations.

Step 5. SAVE OR SUBMIT. If you do not have all of the documents to COMPLETE the application, be sure to SAVE & return at a later time.
