What is care management?
Care Management is a service assisting students in exploring ways to afford basic care needs, such as rent, utilities, food, clothing, medical care, transportation, childcare, etc. The Case Manager will educate the student on services and programming in the community that may assist with basic care needs. The Case Manager will empower and support the student to follow up on referrals and resources to secure their basic and well-being needs.
Example of Care Management
Violet is a second year, independent student, whom has been living off campus. Violet recently received an eviction notice, shut off notice for electricity, and her car was repossessed yesterday. Violet scheduled a care management appointment at the University of Toledo Counseling Center. The case manager educated Violet on rental and utility assistance programs in her county. The Case Manager also educated Violet on the process for obtaining a reduced bus pass. Violet followed up on all resources provided by calling agencies and submitting applications for rent, utility assistance, and a bus pass. After a few weeks Violet received notification, she was denied for rental assistance, but was approved for utility assistance and a reduced bus pass. Violet found the care management appointment to be helpful in learning about resources to assist her as she had never lived independently and was unfamiliar with Toledo, Ohio. Violet was able to communicate with her landlord and together they developed a payment plan. Violet remained in her apartment and now travels to classes by utilizing her bus pass.
