Distance Learning Library Privileges
By completing and submitting this form, I give permission for Interlibrary Loan staff to check out library materials to my library record and ship them to the mailing address I have provided. I also agree to all of the following conditions:
- I am a University of Toledo (UT) distance learning student living more than 50 miles away from either of UT's campuses;
- I do not physically attend classes on either of UT's campuses;
- I will return all borrowed material back to the library, on or before the due date, using the packaging/shipping label provided by University Libraries;
- I am financially liable for any late fees and/or replacement costs associated with non-returned materials, or missing/damaged materials;
- I will monitor my library record for upcoming due dates; and
- I will provide University Libraries with any changes in my mailing address.
