Form 1095-C Available Now
Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) will be available in your MyUT portal beginning 1/30/2026.
Important: Electronic Consent Required Before Accessing/Printing Tax Forms
To view and print your Form 1095-C online, you must first complete your Electronic Regulatory Consent. Ocne this is submitted, you will be able to access the form immediately.
Step 1: Complete Electronic Regulatory Consent

- Log in to the MyUT Portal.
- Select the Employee tab.
- Click Employee Self-Service Dashboard.
- Click Taxes.
- Select Electronic Regulatory Consent.
- Check consent boxes and click Submit.
Step 2: Access and Print Your Form 1095-C

- Return to the Employee Self-Service Dashboard.
- Select Taxes.
- Click 1095-C Employer Provided Health Insurance Offer and Coverage Statement.
- Select the appropriate year.
- Click Display.
- In the lower left hand corner, you can print your Form 1095-C.
