Form 1095-C Available Now

Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) will be available in your MyUT portal beginning 1/30/2026.

Important: Electronic Consent Required Before Accessing/Printing Tax Forms

To view and print your Form 1095-C online, you must first complete your Electronic Regulatory Consent.  Ocne this is submitted,  you will be able to access the form immediately.

Step 1: Complete Electronic Regulatory Consent

    1. Log in to the MyUT Portal.
    2. Select the Employee tab.
    3. Click Employee Self-Service Dashboard.
    4. Click Taxes.
    5. Select Electronic Regulatory Consent.
    6. Check consent boxes and click Submit.

Step 2: Access and Print Your Form 1095-C

    1. Return to the Employee Self-Service Dashboard.
    2. Select Taxes.
    3. Click 1095-C Employer Provided Health Insurance Offer and Coverage Statement.
    4. Select the appropriate year.
    5. Click Display.
    6. In the lower left hand corner, you can print your Form 1095-C.

Electronic Consent Not Completed
