How You Are Billed

We will submit charges to your insurance company for office visits, telemedicine appointments and services provided on site, such as testing, lab draws, some immunizations and medications.

You are responsible for contacting your health insurance company to verify benefits and coverage.

  • Know your benefits and co-pay requirements.
  • Obtain proper authorization for service when required.
  • Submit referral or claims forms if necessary.

Co-pays are collected at time of service. Our student health centers accept credit card payments or cash.

Depending on the services you used during your visit, you could be billed by four institutions. If you have billing questions, be sure to contact the correct agency.

Physicians and providers billed through:

  • University of Toledo Physicians (UTP): 419.383.7197
  • University of Toledo Medical Center (UTMC): 419.383.7400, #2 then #1
