Confidentiality
Medical professionals have a legal and ethical responsibility to protect the privacy of adults 18 years and older. Every visit to our office is strictly confidential.
Information about your visit will not be released to anyone without your written permission. Records are not released except when required by law or when it’s a matter of public health.
How do I give my parents or guardian access to my medical records?
At your first visit, you will be given an Acknowledgement of Privacy Practices and the Confidential Communications Form. The forms will allow you to designate who may have access to your health information. They are updated annually.

Patient Rights
The University of Toledo Medical Center staff is dedicated to caring for each person with respect and dignity. We consider you a partner who wants to understand and make informed decisions about your healthcare. Patient outcomes and participation improve if you know your rights and responsibilities.
You have the right to reasonable access to treatment that is available and medically indicated regardless of age, race, ethnicity, religion, culture, language, physical or mental disability, socioeconomic status, sex, sexual orientation, and gender identity or expression.
Our staff is committed to healthcare equality. You have the right to considerate care that respects your cultural and personal values, beliefs and preferences.
Patient Responsibilities
- Provide accurate and complete information regarding your health history.
- Provide insurance and third-party payer information.
- Satisfy financial obligations for health services provided.
- Keep scheduled appointments or cancel them in advance.
- Follow medical instructions and discuss changes or concerns about your ability to comply.
- Ask questions if you do not understand information about your care.
- Accept the consequences of your action if you refuse treatment or do not follow instructions.
- Act in a considerate and cooperative manner and respect the rights and property of others.
- Follow the rules and regulations of the healthcare facility.
We can provide you a written copy of your patient rights and responsibilities at check in or any time during your appointment.
