Mulford Library Room Reservations

This form is used to request rooms on the fourth and fifth floors of the Mulford Library Building. Reservations can only be made by UToledo faculty and staff on behalf of a university department, unit, or program. Student workers making requests on behalf of their employer must use supervisor's contact information for acceptance. Requests must be made a week in advance whenever possible to avoid disruption to student study.
