Survey Procedure

Requests to survey 500 or more faculty, staff or students should be submitted to the Survey Committee through the Survey Coordination Form on the Institutional Research web page ideally at least four weeks before the proposed survey begin date.

The Survey Committee is appointed by the Provost.

Composition: The committee is composed of two co-chairs with at least five members, including faculty and staff from both campuses.

Meeting Schedule: The committee will hold meetings on a monthly basis as needed and conduct other business via email. Individuals submitting survey proposals may be invited to Survey Committee meetings to answer questions about the project.

Proposal Review and Decision Process: The Committee will review proposals within two weeks of submission. The Human Research Protection Program member of the committee or a proxy from that office will decide if the proposal requires further IRB review. The Survey Committee will then render a decision based on a quorum of 80% or greater. If a quorum of 80% cannot be reached due to committee member absence or disagreement, the survey committee co-chairs will make a final determination. If they cannot come to a decision, they will consult the Provost. The Survey Committee will make one of the following determinations:

Pending with submission of satisfactory revisions

Requires IRB review
