Workload Instructions

General information

Make instructional assignments are updated on the Banner course schedule before you begin.

Click link here to start a workload form

Will require your rockets.utoledo.edu credentials on initial login

Information at top is for individual submitting form (requester)

Enter department chair’s @utoledo.edu email address

    • Enter associate dean’s @utoledo.edu email address for College of Law

Enter academic year and faculty Rocket number

Faculty name and information will appear after a moment if Rocket number is correct

Select dean from drop down list

Document will be forwarded to chair then dean for approval after submission

    • Provost can edit chair’s email address if the incorrect email is entered
    • Provost can change dean if incorrect dean is selected

Teaching activities

Formal / didactic and credit-bearing courses should appear under Teaching Activity section

    • Fall / Spring courses obtained from the course schedule on Banner
    • Summer courses are only included for 12 month faculty
    • Make sure instructors are updated in Banner before creating workload forms
    • Missing in load courses should be entered in Other Teaching Activities section below

Select “Remove” checkbox if listed course is outside of teaching workload

Select “Overload” checkbox if course is being taught as overload

Enter any terms for sabbatical or other planned leave

Select Add Add’l Row for multiple semesters of leave

    • Only enter Fall / Spring sabbaticals and leaves for 9 month faculty
    • Enter Fall / Spring / Summer sabbaticals and leaves for 12 month faculty

Enter independent studies / thesis / dissertation under Other Teaching Activities

Utilize overload section for any courses that will be compensated as overload

Other activities

Use Other Teaching Activities for teaching outside of formal / didactic instruction

    • Ex: mentoring students, writing textbooks, developing curriculum
    • Consult departmental or college elaborations for examples
    • Include any in-load courses omitted under the Teaching Activity section

Also have entries for Professional Activities and Service

Utilize left side to summarize planned activities in year covered by workload form

Utilize right side to summarize outcomes of activities listed as planned in previous workload

Note that all text entry boxes are limited to 2500 characters

    • Briefly summarize activities, Provost does not require details
    • Departments or colleges can maintain paper forms if more detail is required

Submission

Enter workload percentages at bottom of form

Must add to 100% to submit or save

Add any comments in Request Comments

    • Ex: space for faculty to indicate whether they approve or disapprove of workload

Hit Next button to sign

Enter first and last name on page that appears

Have option to download form as PDF for records

Can use these PDFs for public workload display per CBA

Will receive email for successful submission

Saving documents

Document can be saved at any time

Scroll to the bottom enter workload percentages that add to 100%

Can always change these values later if needed

Save Progress button will appear once valid percentages are entered

Will ask if you want to logout or continue

Can log out and come back later

Retrieving saved documents

Login to https://myforms.utoledo.edu

Click Pending / Draft forms

Identify form to continue editing, form will have Incomplete status

Can search for form by faculty name or Rocket number at top of page

Select Action → Complete form to return to form

Chair/Dean/Provost approval

MAKE SURE WORKLOAD DISSEMINATED FOR 3 DAYS PER CBA

Will receive an email with link to complete form Or can login to https://myforms.utoledo.edu

Click Pending / Draft forms

Identify form to continue editing, form will have Draft status

Can search for form by faculty name or Rocket number at top of page

Select Action → Complete form to return to form

Hit Next button at bottom of form to sign

    • Can enter comments before approval

Enter first and last name on page that appears

Have option to download form as PDF for records

Can use these PDFs for public workload display per CBA

Will receive email for successful submission

Chair/Dean/Provost return for revision

Will receive an email with link to complete form Or can login to https://myforms.utoledo.edu

Click Pending / Draft forms

Identify form to continue editing, form will have Draft status

Can search for form by faculty name or Rocket number at top of page

Select Action → Complete form to return to form

Hit select to Return for Revision at bottom of form

Enter email subject and text to return to previous level

Revising the teaching activities section

When a faculty member’s instructional assignment has been updated on Banner

Return the form to the requestor for revision

The requestor re-enters the same Rocket number for the instructor

The course list should update from Banner

The requestor submits the revised form for approval
