Faculty Resources
The Credit by Assessment form should be signed by the student and given to the UT faculty member during the initial
meeting on portfolio development. The UT faculty member retain the form until the
completed portfolio is assessed. The grade, awarded by the UT faculty member, will
be submitted on the Credit by Assessment form to the PLA Coordinator. The student
is given credit for the portfolio in the term the credit was earned.
