intake paperwork

Before you have your intake session, you will need to complete some paperwork online so you understand your rights as a client and the nature of services provided by the clinic. You can access the forms via the email sent to you from your therapist or use this link to see the online forms

Instructions

  1. Go to “Informed Consent Form – Dr. [Name of Supervisor]”. Be sure to select the correct supervisor; this information will be provided in the email from your clinician. You will be prompted to give your first and last name and birth date, so we can match your forms to your clinic records. Please be sure to fill in all the required fields. Once you are done with these forms, you will be given the option to return to the Main Page.
  2. Click the “Main Page” button and go to “Intake Forms.” Please complete these forms and all required fields as well.  Once you are done with these forms, you will be given the option to return to the Main Page.
  3. Click on “Telepsychology Consent” (This is optional but recommended). We recommend completing this form as well in case you would like to have the option of virtual sessions with your clinician.

contacting your therapist

All therapists in the Psychology Clinic are student clinicians who also have classes, other clinical work, and research activites. The clinic office is open 9 AM to 5 PM all days and telephone messages may be left at (419) 530-2721 with the Clinic Manager.  Your therapist will try to return your call promptly, but we cannot guarantee the return call will be in the same business day. 

paying for sessions

You can pay for sessions on our online portal. If you have questions regarding your bill, please email Craig Jeffries at craig.jeffries@utoledo.edu. Payment is due at each session. If payment for more than three sessions is owed, we will not be able to schedule a next appointment.

Therapy clients: Click on the “Therapy” box and put in the amount owed.  You can then “Add to Cart” and check out using your credit or debit card information.

Assessment clients: Click on the “Assessment” box and put in the amount owed.  You can then “Add to Cart” and check out using your credit or debit card information.

telehealth sessions

For virtual sessions, your clinician will send you a link via email for Cisco Webex, which is a HIPAA-compliant platform. Please note that we cannot use any other method of communication for virtual sessions (e.g., FaceTime, Zoom). We encourage you to save the link, which will be used for all your sessions. Webex will work on your browser, but if you'd like to download the Webex app for ease of use, please visit their downloads page.

Canceling and rescheduling

If you need to cancel or reschedule a therapy or assessment session, please contact your therapist as soon as possible and at least 24 hours before your appointment time. If you miss an appointment or cancel with less than 24 hours’ notice, you will be charged the full fee for the missed session.

clinical emergency

In the event of an emergency, dial 911 or go to your nearest emergency room. The National Suicide and Crisis Hotline is 988 (call or text). Do not call the clinic; we do not have the resources to handle emergency situations.
