Forms

 

Faculty may email completed request forms and tests to TestCenter.MC@utoledo.edu or drop off to the Memorial Field House, room 1080, during business hours. Tests and forms are required a minimum of 24 business hours in advance of the expected test date, excluding weekends.*

*To better serve you and your students, it is suggested that mid-terms and finals be sent at least one week in advance whenever possible, as this is our highest volume time.

We require a form for every student for every test. Please assure that you are using the correct form for your test type and fill in all required fields. We cannot administer a test without the proper request form. If your test is a combination of computer-based and paper test, we require BOTH forms.

Paper tests must accompany paper test request forms at submission.

These forms are only to be used for make-up tests or testing accommodations at the Main Campus Testing Center. All regularly scheduled exams and tests should be administered in class. This includes online courses that require in-person tests. We do not have the staffing or capacity to administer whole-class examinations. If your online course requires proctored tests, please arrange an on-campus room and time with your department and/or classroom services.

Off-site proctoring is also available for online classes that require in-person testing. This service is to be used only for major assessments (i.e. midterms and finals) for CCP classes or online students more than 50 miles from campus. Forms are due within the first three weeks of the semester (first week of summer sessions).
