Approved Animal Vendors
Form Location and Instructions
It is UToledo policy that DLAR purchases are handled by DLAR staff. The DLAR staff provides animal procurement services to support your research at The University of Toledo.
What DLAR Does
- Provides quotes for planning and budgeting
- Orders animals from approved vendors
- Confirms order details and funding
- Notifies you about delivery, delays, and animal location
Order Deadline
- Submit orders by Wednesday at Noon for delivery the following week (if available)
- Late orders will either:
- Be placed the next week, or
- Have an administrative fee ($32/hour, charged in 15-minute increments)
Before Placing Your Order, DLAR Checks
- Animals are approved on your protocol
- The Banner index is valid, not expired, and has funding
- Vendor and animal details are correct
After Submission
- DLAR will email the PI/lab to confirm approval, funding, and order timing
- DLAR will update you on delivery status and animal location
Important Requirements

- 3-day acclimation period (required): Do not use animals during this time
- New requisition form (required each time): Do not reuse forms
- Banner index required:
- Orders without a Banner index will not be placed
- Costs will be charged to the provided index after purchase
Approved Vendors and Scheduled Delivery Day
Holidays may cause a change in delivery. DLAR will inform your lab of any changes in scheduled deliveries. If your vendor is not listed here, it is not an approved vendor. You will need to follow the procedures for the Importation of Live Animals.

- Animal Requisitions are due Tuesday by 4:00 PM. Animal orders are placed on Wednesday to deliver the following week.

Instructions
- Fill out the form completely. All spaces above are required to be filled in, if applicable.
- Incomplete forms will cause delays in placing your animal orders.
- You must save the form as your own. You cannot save it in the Z drive folder.
- Send your completed requisition as an attachment to the email addresses listed on the form.
- Animal orders are placed by DLAR with the vendors on Thursday mornings.

"All animals must be acquired lawfully, and the receiving institution should ensure that all procedures involving animal procurement are conducted in a lawful manner. Transportation of animals is governed by a number of U.S. regulatory agencies and international bodies. Movement of animals within or between sites or institutions should be planned and coordinated by responsible and well-trained persons at the sending and receiving sites to minimize animal transit time or delays in receipt."
