Animal Transfer
This form is used to move animals between protocols. Any research staff can submit it, but all PIs must be aware of the transfer.
Important rules
- Animals can only have procedures that are approved on their protocol
- Do not perform procedures that are not listed on the protocol
- If a procedure is approved under a different protocol, you must transfer the animal
first
- Example: breeding protocol → experimental protocol, or between PIs
Billing and PI information
- If two PIs share a protocol, list both names as: PI protocol # / PI paying
- Example: Rat/Mouse
- Rat = PI who owns the protocol
- Mouse = PI who pays per diem charges
- Example: Rat/Mouse
- This ensures the correct PI is billed.

Instructions
- Complete the Animal Transfer Form located in the Z drive folder listed above.
- Submit the Animal Transfer Form via email to the addresses listed below.

- After submitting your form, a transfer label must be placed on each cage card over top of the old protocol information (see example below).
- Blank transfer labels are found in the black DLAR binder in each animal room.
- DO NOT place transfer labels anywhere else on the cage card. The transfer label must cover the old protocol information.
- DO NOT cross off an old protocol number and re-write a new one.
