Federal Work-Study Employment Paperwork

Have you previously participated in work-study during law school?

  • Contact OPD to determine whether you are required to complete any documents beyond the Work-Study Contract.
  • If you have an approved exemption on file, you will automatically return to exempt status following return from non-exempt status. If you are hired in a non-exempt status and wish to opt-out once in an exempt status, you should still apply for the exemption using the app.

Required Documents

Required documents for your employment paperwork will vary depending on your class enrollment during the work-study period. Review the list below and only complete the documents that apply to you. 

Federal Work-Study Contract

Student must complete fields with entries below. This document will be emailed to you from financial aid. It will state your total semester award and will need to be signed by your supervisor and yourself. Once it is signed by both your supervisor and yourself, return the completed form to OPD.

The top section of the contract should be filled out using the template below:

Location = Workplace name where you will be working
Phone # = 419-530-2851
Job Categories =
If on campus, please select the appropriate type.

If working at an off-campus organization, select:
“B ____ Community Service”
Supervisor Information = Must include name, signature, e-mail, and date signed by the community or other on-campus supervisor.
Liaison Information = Completed by OPD. Please leave blank.
Student = Student must sign and date student statement (and Summer Community Work-Study Statement, if applicable).

Acknowledgment of Receipt of Auditor of State Fraud Reporting System Information

Form I-9 Employment Eligibility Verification 

  • Section 1 – Student must complete.
  • Section 2 – OPD completes.*
  • Section 3 – OPD completes if necessary.

Students will receive an email containing a link to complete Section 1 of the I-9.

* Student must bring original identification documents to OPD so that the staff member signing the paperwork on behalf of the University can certify that the documents appear genuine. Please review the list of acceptable documents on the last page of the I-9 form. Student must bring one document from List A OR one document from List B AND one document from List C.


Next Steps

Once your paperwork is processed by Payroll and your "Employee" tab appears in myUT you must electronically complete your W-4 form and Direct Deposit. You will also be getting a text message with instructions from the Ohio Public Employee Retirement System (OPERS) that requires a response.

Employee’s Withholding Exemption Certificate (Ohio or Michigan)

  • Completed online in the myUT portal once you have an employee tab. 
  • If a student claims exemptions, it may reduce the taxes taken out of each paycheck and may result in additional tax liability depending on the student’s tax status.
  • For Ohio, school district codes must be completed and can be found on tax.ohio.gov

Personal History Record (OPERS)

Students who are NOT enrolled in classes for summer and are participating in Work-Study must enroll in the Ohio Public Employees Retirement System (OPERS) by completing this form. 10% of your gross wages will be deducted.

  • Completed online in the myUT portal once you have an employee tab. 
  • Section 1 - Student must complete all blanks.
  • Section 2 - Student must complete all blanks; Job Title = Intern
  • Section 3 - Student must complete all blanks.
  • Section 4 - Students must sign and date.
  • Section 5 - Completed by Payroll. 

Statement Concerning Your Employment in a Job Not Covered by Social Security (OPERS)

  • Completed online in the myUT portal once you have an employee tab. 
  • This must be completed by students enrolling in OPERS.
  • Student completes the following: Employee name; put social security number in "Employee ID #"; sign and date.
  • Payroll completes the following: Employer Name and Employer ID.
