Account Authorization Form
This form must be submitted each time you change your Banner Index Number on file with the UToledo DLAR. Please complete all required information and if applicable non-required information. Once submitted, your information will be received by the DLAR Business Office.
In the event the information below changes, it is the IACUC Protocol Principal Investigator's responsibility to submit a new Account Authorization Form.
