DLAR provides animal procurement services to support your research at The University of Toledo.
- Deadline: Orders are due every Wednesday by Noon to be received the following week (pending availability).
- DLAR will confirm that there is availability on the protocol.
- DLAR will confirm the Banner index provided is associated with the PI on the request, that the index has not expired, and funding is available.
- DLAR will confirm with the PI and laboratory personnel that the protocol availability and funding exists and when the order will be placed with the vendor.
- The PI and lab personnel will be informed when the order has been placed and if there are any difficulties with the order.
- DLAR will notify the lab of any delivery delays, when the animals are received and where they are located within the facility.
- Acclimation Period - Mandatory: A 3-day acclimation period is required before working with any newly received animals.
- New Animal Requisition Form - Required: A new Animal Requisition form should be used from Z drive folder each time you purchase animals. DO NOT SAVE THIS FORM FOR REUSE. This form is subject to change without notice. You are required to use the current version of the form that is active located on the Z drive. Orders submitted on the incorrect form will be returned for resubmission and the deadline will still be enforced.
- Banner Allocation: Once animals are invoiced by the vendor and the order is posted to the DLAR Business Manager's P-Card, the order will be allocated to the Banner index number provided on the requisition. Requisitions turned into DLAR without a Banner index number will be returned as incomplete and not placed with the vendor until a completed form is received.

Instructions
- Fill out the form completely. All spaces above are required to be filled in, if applicable.
- Incomplete forms will cause delays in placing your animal orders.
- You must save the form as your own. You cannot save it in the Z drive folder.
- Send your completed requisition as an attachment to the email addresses listed on the form.
- Animal orders are placed by DLAR with the vendors on Thursday mornings. See above for delivery schedule.
