Inventory Requirements

Inventory Icon Listed here are the inventory requirements for handling controlled substances and dangerous drugs in research. This includes inventory logs and recording requirements.

A. Complete inventory of all controlled substances and dangerous drugs (non-controlled prescription drugs) must be taken at least annually using a University of Toledo Inventory Log or similar template log.

B. The inventory of Schedule I and Schedule II substances must be completed on a separate log from the inventory of Schedule III-V substances.

C. In finished form, the inventory log must include:

  1. Current inventory date
  2. Name of the registrant (or name of PI for dangerous drugs)
  3. DEA number and Ohio TDDD number of registrant, if applicable
  4. Schedule of the substance, if applicable
  5. Name of the substance
  6. Container unit type (i.e., vial)
  7. Container quantity
  8. Container volume
  9. Concentration

D. For substances not in use (i.e., waste or expired substances awaiting disposal/destruction), inventories should include:

  1. Current inventory date
  2. Name of the registrant (or name of PI for dangerous drugs)
  3. DEA number and Ohio TDDD number of registrant, if applicable
  4. Schedule of the substance, if applicable
  5. Name of the substance
  6. Total quantity to the nearest unit
  7. The reason the substance is being maintained by the registrant
