These forms are required for all minors participating in University of Toledo Department of Music ensembles, camps and workshops. They require the signature of the child's parent or guardian who completed the form. Please fill in the form below and print it out. BOTH FORMS MUST BE COMPLETED AND SIGNED BY THE FIRST DAY OF THE EVENT OR YOUR CHILD CANNOT PARTICIPATE! So complete the forms, print them and bring them with you when you bring your child to this event.
