Preclinical Excused Absence Request Form

This online webform must be filled out to request an absence from an examination, quiz, required lecture, small group, laboratory, or a or other mandatory course activity in the preclinical years (MS1 and MS2).

Letter of Good Academic Standing

The letter request form can be found on the Registrar’s Office site at http://www.utoledo.edu/offices/registrar/forms.html.  Students must complete and sign and then return it to the Registrar either by email, fax or mail. Form must include the student’s signature.  - Download here.


Schedule Modification Form

One form must be filled out to:

  • Request a leave of absence for personal, medical, or academic reasons
  • Request an extension of a leave of absence
  • Request to return to school following a leave of absence
  • Modify a required clerkship schedule

The form must be submitted a minimum of 60 days prior to requested modification.
