AFFILIATION: CONNECTING IS AS EASY AS 1, 2, 3!

  1. Complete our online New Student Application
  2. Submit documentation from your high school, another institution of higher education, and/or health care provider or other professionals, which verifies medical condition(s) and explains disability barriers to the educational experience
    • Although many different types of disability related documentation may be acceptable, for your convenience you may ask your health care provider to complete our disability verification form.
    • Please see ADR Documentation Guidelines for additional information.
    • An email will be sent to your Rockets account once information you submit has been reviewed so please check your Rockets email regularly.
  3. Since your input is critical, you must complete a disability access initial appointment with an ADR Accessibility Specialist to set up accommodations and receive training on the procedures for utilizing the accommodations. Once accommodations have been established, you will select the appropriate accommodation for each class for which you are enrolled through ADR’s online system.

Accommodations are typically not retroactive. To utilize accommodations, you must make formal requests in our Disability Accommodations Portal (DAP) system in order for instructors to be properly notified. The University of Toledo Office Of Accessibility and Disability Resources will also consider the unique characteristics of the course, program, or technical standards in order to determine whether an accommodation is reasonable.

Student Referral Resources

Testing must be completed by a qualified health care professional and meet Documentation Guidelines. Students bear the responsibility to pay all costs associated with testing. Students should contact their insurance company prior to making any appointments as your insurance may or may not cover the cost these costs.

Students can choose to use campus resources, their current health care provider or choose a community provider from the list below. This is not a comprehensive list of community resources.
