Common Forms
Which forms do I need?
Students may obtain information on required financial aid forms and requirements by logging into the myUT Portal, and following the steps below:
- Log into the myUT portal.
- Click the "Student" tab.
- Locate the "My Financial Aid" category.
- Click "Check Eligibility Status".
- Click "Student Requirements" and the appropriate aid year.
The outstanding requirements will be listed with links to submit by mail, fax and in person.
Form and document submission methods
Please be sure all forms and documents include the student's name and Rocket Number.
Upload to myUT: Under "My Financial Aid", click "Secure Financial Aid Document Upload".
