Student Organization Travel Request Form (non-club Sports)
A travel request form must be completed by a student or the UToledo advisor of a student organization traveling, at least 2 weeks prior to your scheduled departure. To complete this form, you will need:
- Information regarding your travel (date(s), description, location, etc.)
- Excel sheet of all travelers: Full names, Rocket numbers, and emergency contact information (phone number and relation to the traveler)
- Liability waivers: each traveler needs to fill one out, but all should be uploaded as 1 pdf of scanned documents. Paper copies will not be accepted.

Club Sports Travel
Students who are traveling on behalf of a club sport must complete the following form:
International Travel
If you are traveling out of the country, please also register your trip with the Center
for International Studies and Programs (CISP), using the International Travel Registry.
