New Hire Forms - College of Medicine and Life Sciences
The forms below are required to be completed as a new faculty member at The University of Toledo. Upon completion of the forms, please return to the Office of Faculty Development & Affairs.

Please call 419.383.5000 to schedule your drug screen and immunization appointment. Let them know that you are a new hire.
Required Document List prior to hire
- Background check authorization - This form must be completed and returned immediately as the offer is contingent on a successful background check.
- Clinical Access Form - Clinical Faculty only
- Commitment to Teaching Attestation Form
- Educational and Clinical Care Conflict of Interest Disclosure Form
- Original official transcript of terminal degree. Copies are not accepted. This will be verified through the Medical Staff Office for faculty with medical staff privileges.
- I-9 Employment Eligibility Verification will be completed through an email to the new hire from "Compliance Form I-9" - original documents must be presented to the Office of Faculty Affairs and Development or the department admin. Acceptable documents.

- W-4 Federal Tax Withholding Online Instructions - Completed through MyUT portal

- Inventions and Reporting Statement - Please review the Patent Policy prior to completing this form.
- Safety Signature Form - Please review the Safety Tips at a Glance document prior to completing this form.

Once you receive your Rocket ID, you can use these instructions to activate your UTAD account.

ID Badge - Twenty-four hours after you have activated your UTAD account, you may go online to MyUT to request your ID Badge. Click on the Employee Tab - Other Services - Request New/Replacement Rocket Card. Parking - Once you have activated your UTAD account, you may go online to MyUT to register your vehicle. Click on the Employee Tab - Other Services - Request New/Update Parking Permit.
