The forms below are required to be completed as a new faculty member at The University of Toledo.  Upon completion of the forms, please return to the Office of Faculty Development & Affairs.

Please call 419.383.5000 to schedule your drug screen and immunization appointment.  Let them know that you are a new hire.

 When you get to campus

Once you receive your Rocket ID, you can use these instructions to activate your UTAD account.

ID Badge - Twenty-four hours after you have activated your UTAD account, you may go online to MyUT to request your ID Badge.  Click on the Employee Tab - Other Services - Request New/Replacement Rocket Card.  Please contact Auxiliary Services at rocketcard@utoledo.edu or 419.530.5842 for questions and issues regarding this process. 

Parking - Once you have activated your UTAD account, you may go online to MyUT to register your vehicle.  Click on the Employee Tab - Other Services - Request New/Update Parking Permit.  Please contact the Parking Services at info@parkutoledo.com or 419.530.4100 for questions and issues regarding this process.  Click here for additional instructions.

If you elect to participate in the ARP, you will need to complete this form at the link below, return to Benefits@utoledo.edu, then reach out to the plan contact in the link below to set up your account. https://www.utoledo.edu/depts/hr/total-rewards/retirement/docs/retirement-plan-election-form.pdf.

If you elect to participate in the state retirement system (STRS), you will not need to complete this form. You will need to contact STRS to receive access to the online account. They can be reached at 1-888-227-7877.
