Application for Medical Withdrawal

A medical withdrawal may be requested in the event of catastrophic/serious illness, injuries or conditions that seriously impair and/or incapacitate the student and their ability to attend classes.  Applications based on personal illness/injury require documented medical information. 

The deadline to submit an Application for Medical Withdraw is the last day of the semester in question.  Late applications can not be processed.  Applications are made available only after the deadline has passed for students to withdraw themselves for a particular semester.
