Please Share Some Information About Your Nominee

Do you know UT's next "Champion" honoree? The new Jefferson Awards program recognizes one member of the UT community each month for exemplary volunteer service.

We need to know about the champions within our community. Do you know someone who contributes to the prosperity of a local or global organization? Do you know a student whose generosity has enriched the lives of one - or more - people? Tell us about them!

    Nominating is easy.

Fill in the name, address, and telephone number of your nominee and yourself as the nominator. Write a brief compelling (250 word) story about your friend. Tell something that distinguishes them as an individual. Then highlight how the nominee's actions benefit the lives of others and the local community. That's it!

Step 1: Nominee Information Required on the individual being nominated for a Jefferson Award.

Step 2: Nominator Information Required on the individual who is making the nomination.

Step 3: How has this nominee positively affected the community? (Please limit to 250 words)

Step 4: Community impact besides volunteer commitments of the individual being nominated, please provide how the individual has positively impacted the community. (Please limit to 250 words)
If you see this, leave this form field totally blank!

    Tips on Nominating a Champion:
Honorees are chosen for outstanding acts of service toward their community, charitable organizations and individuals. Think "unsung hero" who donates their time and talents to better the lives of others.

The Jefferson Awards Committee reads every nomination with the following considerations in mind:

    -

Is this person a member of the UT community (student, faculty member, employee)?

    -

Has this person acted in the true spirit of a Jefferson Award Champion honoree?

    -

Does the nomination give a good picture of the nominee's service? When in doubt, give details! The committee gets a better idea of the nominee's dedication if the nomination form reads, "Was chairman of Organization ABC's annual fundraising event that raised $10,000" as opposed to "Is involved in Organization ABC."

    -

Does the nominee serve as part of his/her daily job duties, or as part of the nominee's personal commitment to community?

The selection committee acknowledges all nominations. If you don't get an email that thanks you for your nomination, it may have lost its way in cyber space. Please try again, or email it to kimberley.harvey@utoledo.edu.

Remember - All nominations remain in a pool that is considered each month, so your nominee may win a few months after you let us know about his/her commitment to service.
