New Hire Benefit Information
Welcome to The University of Toledo! Below you will find links to important benefit information.
Please follow these steps to ensure that your benefits are processed:
1.) Submit online enrollment in your myUT portal within 30 days of hire or qualifying event
2.) Submit documentation required for spouse and dependents
a.) Enrollment/changes are not processed until all documents are received
3.) Submit life insurance form within 30 days of hire
4.) Submit retirement plan election form within 120 days of hire
New Hire Orientation PowerPoint Presentation-Plan information will be reviewed at orientation, so bring questions you may have with you.
Life Insurance Enrollment Form- Use this form to provide Beneficiary designation and additional/dependent life elections if applicable. Please select the applicable form.
