Re-engineered Performance Evaluation Tool: The tool evaluates employees on the five (5) pillars, which is identical to the highly successful Recognition and Retention disbursement program and designed to support the University’s strategic plan, Directions, 2011.
“Core Responsibilities” are weighted at 50% with “Customer Service Excellence” at 20% and the remaining pillars of “Nimble and Responsive,” “Professionalism and Respect,” and “Communication Effectiveness” at 10% each. Please include the core duties you outline in your regular performance evaluation under the core pillar listed as “Core Responsibilities.”
These supplemental forms should be used as needed:
- Performance Evaluation -- Individual Development Plan
- Performance Evaluation -- Supplemental Feedback
- Performance Evaluation -- Improvement Action Plan (required if any pillar is scored below a 2)
- Performance Evaluation -- Goal Setting
Tools for CWA Staff Members: Above are the various tools to be used at the present time for all CWA employees. There is NO change in format. Per the collective bargaining agreement (Article 33.3), “Evaluations will be given to bargaining unit members at least at the mid-point and at the end of their probationary periods. Bargaining Unit employees will be evaluated each year within thirty days following their anniversary date (end of probation) and the evaluation shall be signed by the employee to show receipt. If the evaluations are not completed by the due date, they will be considered null and void and not placed in the employee’s file. Timelines may be extended by mutual agreement between the Union and Management.” Due dates for performance evaluations for CWA employees may be accessed via the web report library (instructions below).
To Access the Web Report Library: Go to http://myUT.utoledo.edu sign in, go to the employee tab, and scroll down to the bottom of the page to “Web Report Library.” Click that link, click on the green “Production URL” button, on the left-hand side, click on the “+” by “Public Folders,” click the “+” next to “Human Resources,” then double click on “General Inquiry.” The report you want is titled “CWA Annual Review and Longevity.” Double click on the report and add your parameters. Here are some instructions and support materials regarding the web report library from the Information Technology Department:
